Are you or your loved one needing assistance to remain independent in the comfort of your own home? Are you receiving a Government Subsidised Aged Care – Home Care Package?

Home care package funding is now allocated to you (the client) and will follow you, allowing you to have services that meet your individualised needs and to be serviced by the care service provider of your choice.

HOW TO ACCESS A HOME CARE PACKAGE

Talk to us. We will navigate the process for you and with you. This includes the following steps:

  1. Accessing a free assessment by a member of an Aged Care Assessment Team (ACAT). Contacting My Aged Care Contact Centre on 1800 200 422.
  2. The ACAT team will write to you to tell you the outcome of your assessment and specify the home care package that you are eligible for: Level 1,2,3, or 4.
  3. Once you have been approved for a home care package, you will be placed in the national queue.
  4.  
  5. Prioritisation in the queue is determined by your needs and the date that you were put in the queue. This allows for a fairer allocation of packages.

HOW MUCH IS YOUR GOVERNMENT SUBSIDY?

The Government will contribute the below amounts to your services, to help you to remain independent in your own home:

Level 1 Package $8,045.00 / PA
Level 2 Package $14,837.00 / PA
Level 3 Package $32,171.00 / PA
Level 4 Package $48,906.00 / PA

DO YOU HAVE A HOME CARE PACKAGE BUT WANT TO CHANGE PROVIDERS?

Allow D.A.N.S. to help you navigate this process. If you are unsatisfied with the care you are receiving from your current provider and would like to move over to D.A.N.S., we can assist you in making this transition.

These steps include:

  1. Contact My Aged Care to reactivate your ACAT reference number;
  2. We can liaise with your current provider on a cessation date of service and the transfer of your unspent home care amount;
  3. Notify the Department of Health (1800 020 103) of your choice;
  4. Enter a new home care agreement with D.A.N.S.;
  5. Commence services with D.A.N.S.

WHAT SERVICES CAN MY HOME CARE PACKAGE HELP FUND?

D.A.N.S. can provide up to 24hours or live in care. Your Government funding can contribute to your individual specialised care. D.A.N.S. can also organise your package to contribute to other services including allied health services such as physiotherapists, occupational therapists, podiatrists and more.
